This most recently occurred on June 27, 2018 on the jobsite located at 2806 S. 187th Plaza, Bldg 18661, Omaha, NE 68130. One employee was observed using a hand-held pneumatic nail gun to install roofing material without any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on June 27, 2018 on the jobsite located at 2806 S. 187th Plaza, Bldg 18661, Omaha, NE 68130. Eight employees were re-roofing an existing three-story apartment building. The roof has a five in twelve steep sloped pitch at heights greater than six feet above the ground. H&E Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1245986, citation number 1, item number 1, and was affirmed as a final order on August 24, 2017 with respect to a workplace located at 14513 28th Circle, Bellevue, NE 68123.
