29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and flying chips: a) Mechanic Shop: On or about June 19, 2012, and at times prior thereto, the Dayton Drill did not have a guard at the chuck exposing workers to rotating hazards including dust particles and flying metal when used for making parts in maintenance tasks.

29 CFR 1910.303(b)(1): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations: a) Security Shack - Top Deck: On or about June 15, 2012 and at times prior thereto, the Intech Model 115 electrical power supply cord had exposed wires and damaged insulation. The outer sheathing was pulled away from the wires.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: a) Portside Forward: On or about June 19, 2012 and at times prior thereto, a relocatable power tap was daisy chained to the Air King Fan to provide cooling to the area. b) Forward Change Room: On or about June 19,2012 and at times prior thereto, a relocatable power tap was daisy chained to the ABSCO cold refrigerator. c) Portside Midship: On or about June 15,2012 and at times prior thereto, a relocatable power tap was daisy chained to the vending machine in the aisle.

The Chemical inventory lists various corrosives.
