29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    At the project, on or about June 20, 2012, and times prior thereto, appropriate gloves were not used while an employee was applying wet Portland cement.  Employee was exposed to the hazard of skin contact with wet Portland cement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At the project site, on or about June 20, 2012, the 4 foot step ladder did not extend at least 3 feet above the utility vault. The ladder was extended about 9 inches above the vault.  Employee used the ladder to access the utility vault to perform masonry work.
