29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: At the project, on or about June 20, 2012, and times prior thereto, appropriate hand protection was not worn when applying Portland cement onto the wall. Cotton gloves were used but, did not prevent wet Portland cement from contacting the skin. Wet Portland cement can damage the skin because it is caustic, abrasive, and absorbs moisture.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers: At the project site, on or about June 20, 2012, and at times prior thereto, there was no cover plate on the outlet that powered the water cooler. The water cooler was used by all employees at the site.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the project site, on or about June 20, 2012, and times prior thereto, there was no strain relief for the plug on a water cooler. The outside insulation of the cord was pulled away from the plug.
