29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (A) Annulus 2 - on or about 6/19/2012, catwalk was not adequately guarded in that employees could step off into unguarded area sustaining possible injury. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Sequoyah Nuclear Plant, on or about June 19, 2012, employer has not defined procedures to designate a person who has active role, example: entrant, attendant, supervisor. C. Sequoyah Nuclear Plant, on or about June 19, 2012, employer has not defined procedures for summoning rescue and emergency procedures.

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
