29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Thermoforming Room, Vacuum Thermoform Machine #15. Process technician employee(s) bypassed the safety interlock switch on the machine's barrier gate guard in order to enter the machine while running to make "fine tuning" adjustments to the machine such as but not limited to pin adjustments, knife adjustments, form press ramp adjustments, match metal adjustments and chain tension adjustments. While making adjustments, employee(s) were exposed to crushing, pinching, amputation and degloving injury hazards caused by the machine's moving parts. The employer had developed lockout/tagout procedures to be utilized during tooling changeovers on the vacuum thermoform machines, but did not require that they be used during "fine tuning" adjustments. Alternative measures providing effective employee protection were not utilized. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

While making adjustments, employee(s) were exposed to crushing, pinching, amputation and degloving injury hazards caused by the machine's moving parts. The employee(s) did not turn off or shut down the machine prior to performing servicing and maintenance adjustments. Alternative measures providing effective employee protection were not utilized. Abatement documentation required for this item in accordance with 29 CFR 1903.19(d).
