29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: On or about June 14, 2012, at the worksite located at 3101 North 1st Street in Jacksonville, Arkansas, portions of the supported scaffold erected on the back northeast side of the site were not equipped with base plates. This condition exposed employees to falls from elevation and/or a scaffold collapse.
