29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:       At the Jobsite located 409 Angus, Gretna, NE 68028: The employer is failing to ensure that employees engaged in residential construction activities were protected from falls while working at heights greater than 6 feet from lower levels. On or about March 12, 2015; employees of Ruben Gomez Roofing were observed performing residential roofing activities without conventional fall protection while exposed to fall distances greater than 8.6 feet to the next lower level.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located at 409 Angus, Gretna, NE 68028:  The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 6 feet. On or about March 12, 2015  employees were observed exposed to fall heights of approximately 8.6 feet without protective measures such as but not limited to guardrails, safety nets and or personal fall protection systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     On the west side of the structure located approximately at 409 Angus, Gretna, NE: The employer is failing to ensure that a Werner 16 foot aluminum extension ladder used by employees to access the roof extended at least 3 feet past the roof being accessed, was secured with a rigid support to prevent displacement and/ or provide a grasping device to assist during mounting and dismounting the ladder.
