29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.    In the event of an emergency evacuation, the employees were exposed to hazards such as related to a Fire, and Anhydrous Ammonia leaks. The employees were exposed in that sufficient space for an exit discharge was not provided:    a)On the Fab-floor, the Shank saw operator(s) (Saw #17) was allowed approximately 16 inches for egress.    29 CFR 1903.19(d)(1) Requires certification and documentation that the abatement of the above violation is complete.        b)On the Fab-floor, an employee working below a staircase (Between BT4 and BT5) did not have an egress.    c)On the Fab-floor, employees (approximately 19 employees) working in back-pack-off were allowed approximately 17 inches for an egress.    d)On the Fab-floor, the Wing saw operator (Saw #3) was allowed approximately 16 inches for egress, from the stair to the combo box.    e)On the Fab-floor, an employee  working below a staircase (Between BT2 and BT3) was allowed approximately 10 inches on one side and 16 Inches on another side.    29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     On the Kill floor, employees working next to the split-saw platform were exposed to crush-by hazards in that a guard system was not in place to keep employees from walking beneath the split saw platform.

29 CFR 1910.219(f)(3): All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.     In the Rendering Room, employees were exposed to amputation hazards. The employees were exposed in that the chain and sprockets at the Server-tex feed auger and the Server-tex Discharger were not completely guarded.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.    In the Processing Floor at the trim belt a projecting shaft was more than one-half the diameter of the shaft. The diameter was approximately 1 inch and projected approximately 2 inches

In the ABS Maintenance Shop, flex cords were extending through walls located:     a) On the main brick wall energizing a clock.    b) Through a metal cabinet energizing a small battery charger.
