29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel. (a) Bureau of Reclamation, Building 1400, environmental library cubicle 222K, the Hi-Saver multiple outlet surge protector was used to provide power to cubicle outlets and extension cord without a ground fault circuit interrupter for personnel protection.

(a) Bureau of Reclamation, Building 1400, environmental library cubicle 222K; the male end of the flexible electric cord did not have strain relief and insulation was pulled away from the outer sheath.
