29 CFR 1910.27(d)(4): Vertical grab bars did not have the same spacing as the ladder side rails: On or about June 29, 2012, employer did not ensure that grab bars were installed. The inclined ladder leading to the asphalt container and the vertical ladder leading to the asphalt pump were missing the right hand side grab bar. This condition exposed employees to potential falls and struck-by hazards.
