29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about May 05, 2025 at the shop area, the employer did not ensure fall protection was provided when working on top of tank trailer at an approximate height of 12 feet from ground level, as employee(s) are on tanks to verify the manhole is properly closed, doing maintenance and to measure the volume inside the tank.

29 CFR 1910.106(b)(6):"Sources of ignition." In locations where flammable vapors may be present, precautions shall be taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat. On or about May 05, 2025 at the shop area, employees utilized a torch and a metal pole on a tar truck with a tar category 3 flammable liquid without preventing the source of ignition while trying to unclogged a drain in the truck
