On or about 06/27/12, an employee was exposed to a fall hazard of about 30 feet while working from an aerial lift while trimming trees without means of fall protection at 8760 NW 4th Street, Plantation FL. 33324. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION. REPEAT: 4-EVER-GREEN LAWN CARE R.K.P.; INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.67 (c)(2)(v) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312152929, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 20, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 4703 A1A, LAUDERDALE BY-THE-SEA, FL. 33300.
