29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a. On or about 30 April, 2014, at the above addressed job site, an employee was exposed to the hazard of falling while working from an aerial lift without having his safety lanyard attached to his safety harness. REPEAT: 4-Ever-Green Lawn Care R.K.P., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.67(c)(2)(v), which was contained in OSHA inspection number 488158, citation number 2, item number 1 and was affirmed as a final order on 05 December 2012, with respect to a workplace located at 8760 NW 4th Street, Plantation, FL, 33324.
