29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:          Although the employer developed and implemented an energy control program for use at the facility, it was deficient and did not include specific machine procedures, which are used to identify applicable energy sources and the appropriate methods for isolating, blocking, or securing machinery. Equipment includes, but is not limited to; the companys (2) stand mounted routing machines and (3) CNC machines.

The unguarded equipment includes:      a. (2) stand mounted routers;     b. (3) Haas CNC Milling Centers. The access door interlocks were bypassed.

The metal junction box, located in the machine shop- production area was not installed in a neat workmanlike       manner.  The  box was not mounted to a fixed surface nor connected to the conduit.     b. The switch used for turning on and off routing machinery, was a standard household light switch, and is not designed       to prevent unintentional restarting of the machinery and/or constructed and mounted to minimize inadvertent       operation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    On or before 06/26/2012 a workplace hazard assessment was not completed by the employer.  Personal protective equipment such as, but not limited to, safety eyewear, protective gloves or a face shield may be necessary for employees tasked with changing out the propane cylinder on the company's fork truck.
