29 CFR 1926.501(b)(13):     "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.    (a)  200 Lakota Ln Lee's Summit MO 64086.  Seven employees were working on a roof with a 8/12 pitch without fall protection.

29 CFR 1926.451(a)(6):     Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.    Appendix A (k):    Ladder jack scaffolds. Maxiumum inteded load - 25 lb/ft2.  However, not more than two employees shall occupy any platform at any one time.  Maximum span between supports shall be 8 feet.    (a)  200 Lakota Ln, Lee's Summit MO 64086.  A scaffold was install with four ladder to support it and the platform was made of three wood panel bridged.  Three person on that platform can increase the possibility of failure and fall into the floor.

29 CFR 1926.452(k)(5):     Scaffold platforms shall not be bridged one to another.      (a)  200 Lakota Ln, Lee's Summit MO 64086.  A jack scaffold was built with the platform bridged.  The high of the platform was 20 feet.  If one of the ladder fail the employees can fall down from the platform.
