29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 1517 MO HWY 7, Pleasant Hill, MO 64080:  Employees were exposed to a struck-by hazard.  Employees were exposed to injuries to the eyes and the face while using a Hitachi brand pneumatic nail gun without the use of eye protection.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential construction site located at 1517 MO HWY 7, Pleasant Hill, MO: Employees were exposed to fall hazards while performing roofing activities. This was demonstrated when employees were using a WERNER extension ladder to gain access to the upper landing of a roof. The ladder was only placed 2 feet above the upper landing and was the only means of access on and off the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential construction site located at 1517 MO HWY 7, Pleasant Hill, MO: Employees were exposed to fall hazards while performing roofing activities. This was demonstrated when employees were observed working at an elevation of approximately 10.2 feet without the use of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems.   The LARRY VAUGHT ROOFING was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 976902, citation number 1, item number 1 and was affirmed as a final order on June 12, 2014, with respect to a workplace located at 3151 NW 90th St, Kansas City, MO 64154.

29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system: Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.  At a worksite located at 1517 MO 7 HWY, Pleasant Hill, MO: Employees were exposed to fall hazards. Employees were performing reroofing work during which time employees dawned body belts. The body belts were attached to a lanyard and rope, and then to an anchor point. The body belt was being used instead of a full body harness as a means of personal fall protection.

29 CFR 1926.95(d)(1): Except as provided by paragraphs (d)(2) through (d)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employee(s).  The employer is not providing personal protective equipment (PPE) to employees at not cost, when such equipment is necessary to protect employees from job-related injuries, illnesses, and fatalities. The employer is deducting money from employees paychecks on a monthly basis until the total coast of issued personal protective equipment is paid in full. Once fully paid the personal protective equipment belongs to the employee and they are responsible for its care and maintenance. The issue personal protective equipment often come to employees as a kit and the employees bare the cost of the equipment until paid in full at $315.00.
