Ceiling department - during modular building operations, the roof where employee(s) were installing insulation and decking was 4'6" from the concrete slab and exposed employee(s) to a fall hazard due to no fall protection being used. Violation observed on or about 6/28/12. The Frontline Comunications was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23, which was contained in OSHA inspection number 312735111 citation number 01, item number 001 and was affirmed as a final order on January 4, 2009 with respect to a workplace located at 2500 Industrial Street, Leesburg, Florida 34748.
