29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:      On or about June 28, 2012, employees engaged in roofing activities on a apartment building utilized an extension cord to power a walking iron. The power cord was not provided with strain relief exposing employees to electrical shock hazards.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.      On or about June 28, 2012, employees performing roofing activities on an apartment building were not protected from falling more than (20) feet to the ground below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:        On or about June 28, 2012, employees performing roofing work were using an extension ladder that did not extend (3) feet above the landing to access  and egress the roof.  The employees were exposed to a fall hazard of up to (20) feet.
