29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about June 20, 2012, on a roof, where the employer did not ensure that workers were protected from tripping in, stepping into or through uncovered 4x4 mechanical shafts. Inspectors were crossing over ceiling joist assembly beams while being exposed to fall hazards of approximately 22-feet to ground below from the presence of uncovered mechanical shafts located throughout the walking/working surface.
