29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) Craig Fahr, Incorporated, 675 West Main Street, Annville, PA - Employees working on a Biljax fabricated frame scaffold were observed climbing down from the scaffold without the use of ladders, exposing employees to an approximate ten (10) foot fall hazard, on or about June 29, 2012. No abatement or certification required.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system: (a) Craig Fahr Masonry, Inc., 675 West Main Street, Annville, PA - Two (2) employees were laying cinder blocks while working on a Biljax fabricated-frame metal scaffold with missing end rails, exposing the employees to an approximate eleven (11) foot fall hazard, on or about June 29, 2012. No abatement documentation is required.

29 CFR 1926.454(a): The employer did not train each employee who performed work while on a scaffold by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) Craig Fahr Masonry, Inc., 675 West Main Street, Annville, PA - Employees were laying block while standing on an approximate eleven (11) foot high Biljax metal fabricated-frame scaffold and did not receive training on the hazards of working on scaffolds, on or about June 29, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
