29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    At the worksite, on June 22, 2012, the employees conducted maintenance and repairs on various types of equipment such as but not limited to Blanchard grinders and the lapping machines.  This condition exposed employees to caught-by, struck-by and electrical hazards.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:      At the worksite, on June 22, 2012, the employer owned and operated 9 wall and column mounted cranes.  None of the cranes had recieved a periodic inspection, exposing employees to struck-by hazards:     (a)  1/2 ton Handling Systems located on the north wall of the 'Big Shop', unknown model number and serial number.    (b)  1/4 ton Dayton located on the east wall of the 'Big Shop', model number 4zw97, unknown serial number.      (c)  1/4 ton Dayton located on the west wall of the 'Big Shop', model number 4zw97, uknown serial number.     (d)  1/2 ton Handling Systems located along the east wall of the 'Big Shop', uknown model number and serial number.     (e)  1 ton unknown manufacturer along north wall of 'Lapping Shop', unknown model number and serial number.    (f)  1 ton unknown manufacturer along northwest wall of 'Lapping Shop, unknown model number and serial number.      (g)  2 ton unknown manufacturer along west wall of the 'New Shop', unkown model number and serial number.      (h)  1 ton Handling Systems located in the north section of the 'New Shop', unkown model number and serial number.     (i)  1 ton Handling Systems located in the southeast section of the 'New Shop', unknown model number and serial number.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    At the worksite, on June 22, 2012, the Siemens circuit breaker panel on the west wall of the 'Big Shop' had 4 circuit breakers missing. The voids were not covered, exposing employees to electrical shock and electrocution.
