29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a)  At the facility, where employees used elastomeric half-faced respirators when filling the blasting cabinets with Tungsten blasting media.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminantes chemical state and physical form:    a)  At the facility, where the employer had not determined the Tungsten airborne concentration levels in the Spray Building.  Employees wear elastomeric half-face respirators while pouring Tungsten blasting media into the blasting cabinets.    b)  At the facility, where the employer had not determined nor evaluated the airborne dust concentration levels in the Deburr Building.  Employees wear 3M dust masks while deburring the metal pieces.  This condition exposed employees to respiratory hazards.
