29 CFR 1926.102(a)(2): Eye and face protection required by this Part did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:    a)  Bass Builders at 621 Sparrow Place, Fort Collins, CO 80525:  On or before June 30, 2012, an employee who was using a pneumatic nail gun was allowed to use sunglasses for eye protection that did not meet ANSI Z87.1 requirements. Condition exposed this employee to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b).  Note:  If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    a)  Bass Builders at 621 Sparrow Place, Fort Collins, CO 80525:  On or before June 30, 2012, the employer did not ensure that employees were protected from falling in the following residential construction circumstances:    1)  Three window wall openings on the second floor had the bottom ledge 37" above the second floor;  and    2)  One basement window well included a 49" fall hazard.    Condition exposed employees to fall hazards.     Millard, Curt dba Bass Builders was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 313418691, citation number 1, item 2, issued May 6, 2010 and became a final order on June 23, 2010, with respect to a workplace located at 435 Draw Steet, Lochbuie, CO 80526.    Millard, Curt dba Bass Builders was previously cited for a repeat violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 314663618, citation number 1, item 2, issued March 29, 2011 and became a final order on May 18, 2011, with respect to a workplace located at 2984 Ariel Dr., Loveland, CO 80537.    Abatement Note:  29 CFR 1926 502(b)(1) provides that the top edge of guardrails shall be 42 inches +/- 3 inches.

29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a)  Bass Builders at 621 Sparrow Place, Fort Collins, CO 80525:  On or before June 30, 2012, the employer provided an orange and black extension cord, with an open ground, for use in powering portable electrical tools, including a Dewalt circular saw model DW535T (SN not available), and a Dewalt Sawzall model DW304P serial number 214853.  Condition exposed employees to electrical shock hazards.

Condition exposed employees to electrical shock hazards.
