29 CFR 1910.147(c)(6)(ii): The employer did not certify that the periodic inspections of the lock-out/tag-out procedures had been performed. There was no certification completed that identified the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Facility-The company had a written lock-out/tag-out program that required inspections and provided a form to be used to document the inspections. The inspections were not completed and certified as required in the company program.
