29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken: a) Penn Hill High School jobsite, 309 Collins Drive, Penn Hills, PA: On or before 06/25/12, a Sky Trak 36 RT forklift truck was being operated within two (2) feet of three (3) one hundred (100) pound unprotected propane cylinders. The propane cylinders were being used to fuel a 600 Garlock asphalt kettle being operated by an employee at the jobsite.

No. 10A-1970: a) Penn Hill High School jobsite, located at 309 Collins Drive, Penn Hills, PA: On or before 06/25/12, an Amerex Model A411 portable fire extinguisher located on the B roof section of the building was discharged. b) Penn Hill High School jobsite, located at 309 Collins Drive, Penn Hills, PA: On or before 06/25/12, an Amerex portable fire extinguisher located by the Garlock asphalt kettle was not inspected on a periodic basis.
