29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which took the monitor's attention away from the monitoring function: a) Job site, 8950 Duncan Avenue, Pittsburgh, PA: On or about October 8, 2019, during roofing work, the employer did not ensure that the safety monitor had his attention focused on his safety monitor duties in that he was observed doing work such as loading tools, equipment and materials on the roof while an employee worked outside the flag line near the edge of the roof, exposing the employee to a fall hazard of at least approximately 15 feet.
