On or about June 21, 2012 - at the above addressed jobsite, an employee was operating and lifting metal/steel members using a galvanized cable sling placed around the forks of a JLG/GRADALL TELEHANDLER G6-42A Rough Terrain Forklift when this practice is not recommended and or approved by the manufacturer, thereby exposing the employee to struck by hazards due to the possibility of the load breaking free from the galvanized cable sling, or sliding off the forks.
