29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: At a commercial construction job site, employees welded from a shop made platform, that was not an approved work platform by the manufacturer for capacity, construction, use, and fall protection. Employees were exposed to a fall hazard of up to approximately 18 feet to the ground below.

At a commercial construction job site, employees welded from a work platform elevated by a JLG 6042 SKYTRAK that was not secured to the SKYTRAK. The employees were exposed to a fall hazard of more than 13 feet to the ground below and being struck-by and crushed by the work platform.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: At a commercial construction job site, employees welding from a shop made platform were not provided with fall protection. Employees were exposed to a fall hazard of up to approximately 13 feet to the ground below. B&B Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 976785, citation number 1, item number 2 and was affirmed as a final order on June 9, 2014, with a final abatement date of June 4, 2014, with respect to a workplace located at 11120 Northwest Freeway, Houston, Texas 77092
