29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Jobsite - 9402 Section: B01, Aliquippia, PA: On or about June 28, 2012, employees were using an extension cord with exposed wires at the male end of the cord, thereby eliminating the strain relief.
