29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: Instances including, but not limited to the following: a) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about January 4, 2016, during steel erection activities, the employer did not ensure that appropriate fall protection systems/devices were used while performing work such as bay gauging, exposing employees to fall hazards of up to approximately 49 feet. b) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about January 4, 2016, during steel erection activities, the employer did not ensure that appropriate fall protection systems/devices were used while performing work such as welding bridging and related steel/iron, exposing employees to fall hazards of up to approximately 49 feet. c) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about January 4, 2016, during steel erection activities, the employer did not ensure that appropriate fall protection systems/devices were used while performing work such as decking installation, exposing employees to fall hazards of up to approximately 24 feet. d) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about January 5, 2016, during steel erection activities, the employer did not ensure that appropriate fall protection systems/devices were used while performing work such as decking installation, exposing employees to fall hazards of up to approximately 24 feet. Caution tape was put up around the perimeter of a building floor rather than a permissible fall protection systems/devices.
