29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: JOB SITE: 12111 Wesland Drive, Maryland Heights, MO 63043, at this worksite workers were observed on the top of a 3:12 pitch roof, actively installing new roof shingles, using pneumatic nail guns without any form of eye or face protection. The workers were exposed to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: JOB SITE: 12111 Wesland Drive, Maryland Heights, MO 63043, at this work site, workers were observed on top of a 3:12 pitch roof, approximately 12 feet 7 inches eave to ground level, actively installing new roof shingles and were not protected by guardrail systems, safety net systems, or personal fall arrest systems. The workers were wearing full body harnesses, but were not tied down to an anchor point with lanyard. The workers were exposed to a fall hazard.
