Union St. Cedar Grove, WI: On and about November 30, 2017, employees were exposed to fall hazards while reroofing a steep pitch church with a 20' eave height without adequate fall protection methods in place, in that:     A: One employee walked approximately 50' along the ridge without being connected to a lifeline;    B: One employee was working at the eave edge while standing on a roof jack without a personal fall arrest system:    C: One employee was working at the eave edge while standing on a roof jack when he did not have a lifeline;    D: Several employees had their lifelines connected to the harness webbing at their waists when their harnesses were not equipped with side D-rings.

Union St. Cedar Grove, WI: On and about November 30, 2017, employees were exposed to falls while reroofing a church when one lifeline had a knot tied below the snap hook and a second lifeline was tied to the anchor.  Knots are known to reduce the load limits of rope by 30%-80%, reducing a 5400# rated rope to below 5000#.
