29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it was infeasible or that it created a greater hazard to use these systems, and did not develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  Leal, Maynor at 673 Dakota Way, CO 80550:  On or before June 30, 2012, the employer did not ensure that employees were protected from falling when framing in that an employee was working on the exterior top plate when nailing a 2 x 4 to the top plate for use in hanging ceiling sheet rock.  Condition exposed employee to fall hazards of 11.6'.

29 CFR 1926.404(a)(2):  A grounded conductor was attached to a terminal or lead so as to reverse designated polarity:    a)  Leal, Maynor at 673 Dakota Way, CO 80550:  On or before June 30, 2012, the employer did not ensure that an orange extension cord did not have reverse polarity when used with 120 volt power tools including a Skilsaw, model Mag 77, and a Sawzall, model 6509 serial number 808B397300651.  Condition exposed employees to electrical shock hazards.

29 CFR 1926.404(b)(1)(ii):  On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    a)  Leal, Maynor at 673 Dakota Way, CO 80550:  On or before June 30, 2012, the employer did not ensure that the black dual outlet set on the temporary panel included a working ground fault circuit interrupter when used with 120 volt power tools including a Skilsaw, model Mag 77, and a Sawzall, model 6509 serial number 808B397300651.  Condition exposed employees to electrical shock hazards.
