Employee was not wearing a face shield, the tools were not inuslated nor the live busbar deenergized or protected. On or about 7/4/2012.

The employee was installing a new feed to a live 120/208 volt busbar and the tool made contact between 2 of the live phases. On or about 7/4/2012.
