29 CFR 1910.269(e)(5): When covers are removed from enclosed spaces, the opening was not promptly guarded by a railing, temporary cover, or other barrier designed to prevent an accidental fall through the opening and to protect employees working in the space from objects entering the space. Consolidated Edison Company of New York, Inc. - Victory Blvd. & Sheraden Ave., Staten Island NY 10314 - On or about January 11, 2026, a Con Edison employee was exposed to a fall hazard while reinstalling a manhole cover. The manhole opening was approximately 8 feet deep. The employee was not using any form of fall protection to prevent an accidental fall through the uncovered opening. As a result of this condition, the employee fell into the manhole and sustained serious injuries. Incident occurred on January 11, 2026
