29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: On or about 07/02/2012, an employee was removing forms at the support of excavation area on the Dodge Island side of the tunnel when he leaned against the top rail to observe what work had be done and the top rail became unattached from the post; the employee fell approximately 8 feet to the level below. Note: Certification of abatement is required for this violation.
