29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  On or about June 28, 2012, employees were exposed to fall hazards while using a ladder to access a roof where the side rails did not extend at least 3 feet above the roof surface.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally:    a)  On or about June 28, 2012, employees were exposed to fall hazards while using a ladder to access a roof where the side rails of the top of the ladder were placed on a rake edge of a dormer where they were not supported equally.  The entire ladder was also placed at an angle.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    a)  On or about June 28, 2012, employees were not protected from fall hazards in excess of 6 feet while performing roofing work without fall protection.    FOC Roofing was previously cited for a violation of the Occupational Safety and Health Standard, or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 313720393 Citation 1 Item 3a, issued on June 25, 2012 and became a final order on July 22, 2010 with respect to a worksite located at 7480 Terry Court, Arvada, CO 80007.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  On or about June 28, 2012, employees were not provided with training on how to identify fall hazards during roofing work and how to mitigate such hazards.    FOC Roofing was previously cited for a violation of the Occupational Safety and Health Standard, or its equivalent 29 CFR 1926.503(a)(1) which was contained in OSHA inspection number 313720393 Citation 1 Item 3b, issued on June 25, 2012 and became a final order on July 22, 2010 with respect to a worksite located at 7480 Terry Court, Arvada, CO 80007.
