29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  a) Englewood Residence: On or about April 10, 2015, and at times prior, a company manager was on a job site and did not determine a hazard existed when he assigned five workers to perform roofing activities with only three sets of fall protection available for use.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Englewood Residence: On or about April 10, 2015, and at times prior, five workers were not wearing a harness and had no other means of fall protection while exposed to falling more than 14.4 feet to ground level.   FOC Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 544639, citation number 01, item number 003, and was affirmed as a final order on September 28, 2012, with respect to a workplace located at 10935 Valleybrook Circle, Highlands Ranch, CO 80130.  FOC Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 544598, citation number 01, item number 002, and was affirmed as a final order on September 28, 2012, with respect to a workplace located at 8841 Fairway Oaks Way, Lone Tree, CO 80124.
