29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person: (a) 211 and 213 East High Street, Dallastown, PA - Employees were using a ladder jack scaffold that was not designed or erected by a trained person, thereby exposing employees to a fall of 19 feet to the ground below, on or about July 9, 2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
