29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about 7/2/2012, at the Elberton facility, the employer did not establish a Lockou/Tag Out program, with specific written procedures for equipmnet, employee training and periodic inspections to ensure employees performing service and maintenance to equipment, including press brakes, laser machines and DLL machines, were protected from unexpected startup of various energy sources.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    On or about 7/2/2012, in the Service Lift area, employees were exposed to finger, hand and struck by hazards as the Ryobi bench grinder (serial # W0217 36645) was being used without a work rest attached and in place. On or about 7/2/2012, in the welding area employees are exposed to fire and smoke hazards as welding screens do not extend high enough to prevent welding and grinding sparks from landing on cardboard boxes that are located adjacent to the welding/grinding area.
