There shall be no open flame or spark producing equipment in any spraying area nor within 20 feet thereof, unless separated by a partition. a) On or about July 10, 2012 The employer failed to ensure that there was no spark producing equipment within 20 feet of the spraying area. A computer system consisting of a monitor and hard drive were stored directly in front of the opening to the spray booth and plugged into a standard electrical outlet not approved for Class I Division II hazardous locations.

On or about July 10, 2012 the employer failed to ensure the interior surface of the spray booth was smooth and continuous without edges and otherwise designed to prevent pocketing of residues and facilitate cleaning and washing without injury. Cardboard pieces were hung on the interior sides of the spray booth.
