29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Jobsite located at Fraternity Circle, URI Campus, Johnston, RI: On or about 06/27/2012 employees were conducting construction activities near the northeast entrance to the eastern most dorm whereby the employees were exposed to trip and fall hazards from the accumulated construction debris which included, but was not limited to, bricks, waste concrete piles, garden hose, plastic sheeting, wooden pallets, and uneven walking surface.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records required to be maintained under Part 1904 within the four business hours. Located at Total Wall Systems, Inc., 54 North Harbor Street, Branford, CT: On or about 06/27/2012 a verbal request for the FY 2009, 2010, 2011, and 2012 OSHA Injury and Illness Logs was made by the Compliance Officer. On 08/29/2012 a faxed request for the documentation was made whereby the employer failed to provide copies of the requested injury and illness recordkeeping.
