29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    At the project site on or about July 9, 2012, and at times prior thereto, 2 x 4 lumber with protruding nails, rebar(s) and other construction debris lay directly in the pathway to the work area.  The lumber was used as formwork and dismantled sometime prior to this inspection.  Employees were exposed to the trip and impalement hazards.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    At the project site on June 9, 2012, and at times prior thereto, eye protection was not worn while driving and removing nails from lumber. Employee was exposed to metal debris striking the eye(s).
