Instance 1: The exit route was blocked by a large container used to collect scrap metal on July 11, 2012.  Instance 2: The exit route was blocked by multiple pallets and other equipment on August 20, 2012.

29 CFR 1910.215(a)(4):     Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    Delta 8 inch bench grinder, Milling area, adjacent to Supervisor's Office, Rodney Hunt Company: The work rests of the Delta 8 inch bench grinder were not adjusted to a maximum of one eighth inch from the wheel. Using a wooden carpenter measure, the gap was found to be approximately one quarter inch on the left wheel and three eighths inch on the right wheel.
