29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a)Luna Roofing, 1317 13th Avenue, Greeley, CO: The portable aluminum extension ladders (2) used to gain access to the roof did not extend at least 3 feet above the landing surface. This extension ladder had been taken apart (separated) to provide access on both sides of the home. As a result, each ladder extended approximately 6 inches above the gutter line.  Condition exposed employees to a fall hazard of approximately 10 feet.     Abatement Note:  Abatement certification is not required for this item.

29 CFR 1926.501(b)(13)  Employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:    (a)  Luna Roofing, 1317 13th Avenue, Greeley, CO:  The employer had not ensured the use of appropriate fall protection while employees installed roofing shingles.  On or about July 3, 2012, employees were working on a 6/12 pitch roof without appropriate fall protection.  Condition exposed employees to a fall hazard of approximately 10 feet.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").    Luna Roofing, Company was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 280479, citation number 2, item number 1, issued on April 26, 2012, with respect to a workplace located at 36980 County Road 37, Eaton, CO.      Note:  When the employer demonstrates that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a written fall protection plan meeting the requirements of paragraph (k) of 1926.502.
