29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employees performing materials handling with a forklift in the following instances were exposed to struck-by hazards and the employer did not obtain manufacturers prior written approval. 1) Employees were performing metal work on a part which was suspended by a job-made jib addition to a Yale forklift (M# GLC060VXNVSE088, S# A910V11789E). 2) Employees were using a Yale forklift (M# GLC155VXNGJV094.9, S# D879V01569G) with the overhead guard covered by an opaque piece of insulation which restricted the operators visibility.
