29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    On or about July 13, 2012, at this site, employees were exposed to falls of 4 feet or greater to ground while working on the tops of oilfield frac tanks, where temporary guardrails or equivalent protection was not provided. One 13-step mobile stand was missing its right hand rail, and one 10-step mobile stand was missing a guard chain at the top work platform.
