WORKSITE: The following employees were not protected by a fall arrest system/device while working in aerial lifts: 1. The employee welding metal window clips from the Snorkel model TB-66J19 aerial lift. 2. The employee moving the Snorkel model TB-66J20 aerial lift.

WORKSITE: The chain used to secure the telescoping jib lift attachment to the Lull model 1044C-54 rough terrain forklift was not properly secured in place.
