29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded: (a) At the front of the building on or about July 17, 2012, and at times prior thereto, the blade guard of the Rigid TS2400-1 table saw, serial number U072869973, was removed and exposed the operator to amputation hazards. The saw was used to cut lumber to support metal studs.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. (a) At the front of the building on or about July 17, 2012, and at times prior thereto, an assured equipment grounding conductor program or ground-fault circuit-interrupters were not used to protect an employee that used a 110 VAC Rigid TS2400-1 table saw, serial number U072869973, powered through and extension cord. The saw was used to cut lumber to support metal studs.
